I have been asked to find a way to scan a premade paper form, with Radio buttons.. (it can be re-designed). and take the info and put it into digital so that it can be stored in a database..

 I have never been asked this before, so i'm guessing we'll need some type of software that will scan a document then know by the layout of the form where the options are selected or now.

anyone here have any experience with this, or suggestions for me?
